Patient Participation Group - we want you!!
Help influence the development of health services in your practice. We are looking for members to join our Patient Participation Group. Come along and discuss your ideas for the practice, hear about any changes that may affect you.
Our first meeting will take place on Thursday 19th January 2012 at 2pm

Patient Information - This Practice will be uploading YOUR patient record details onto the national computerised system called Summary Care Record Commencing March 5th 2012
Summary Care Records enable medical staff to have access to important medical information in emergency settings about your current medication, allergies and bad reactions to medicines.
If you would like further information, or want to opt-out, please speak to our Reception staff or visit www.nhscarerecords.nhs.uk
The Government require us to collect certain information about our patients. Please could you help us? Next time you contact us or are in surgery (if we haven't already asked you) please could you let us know what ethnicity you are and whether you currently smoke or not. All information collected will be treated in confidence and only used in the interest your individual welfare. Statistical information only will be forwarded at Government request.
